Soil Born Farms Urban Agriculture & Education Project

Soil Born Farms’ mission is to create an urban agriculture and education project that empowers people to participate in a local food system that encourages healthy living, nurtures the environment and grows a sustainable community. Our 55-acre organic farm is a regional center for education about food, health and the environment. Soil Born Farms’ work has elevated awareness of urban food production as a means for improving individual health, neighborhood cohesion and environmental stewardship.

With its holistic approach to health and community development, Hope Farms is transforming the struggling Sunnyside area through grassroots initiatives and environmental awareness.

Project Angel Heart

We prepare and home-deliver meals to neighbors living with cancer, HIV/AIDS, Alzheimer’s, kidney/heart/lung disease, and other chronic illnesses throughout Colorado. The meals we provide are medically tailored as they are approved by a registered dietitian and use evidence-based guidelines to ensure positive health outcomes. We tailor our meals to meet the individual and medical dietary needs of our clients, combining medical diets (i.e. renal-friendly, heart-healthy), with modifications for allergies (i.e. gluten-free, dairy-free, nut-free), side-effects, medications (i.e.

Northern Illinois Food Bank

Northern Illinois Food Bank’s mission is to provide nutritious food and resources for our neighbors with dignity, equity and convenience through partnerships and innovation with a vision for everyone in Northern Illinois having the food they need to thrive. We are committed to increasing access to nutritious food and providing a food assistance experience that is neighbor-centered, convenient and equitable, aiming to serve at least 90% of our food insecure neighbors in our service area by 2025.

Northeast Organic Farming Association Massachusetts Chapter

Through education and advocacy NOFA/Mass promotes organic agriculture to expand the production and availability of nutritious food from living soil for the health of individuals, communities and the planet. Our Organic Food Distribution and Community Gardening Project includes intensive education in organic food production, and also distributing organic food through Open Pantry in Springfield Massachusetts. NOFA/Mass already partners with many Springfield organizations, particularly communities who are food insecure and living in food deserts where access to fresh food is unavailable.

Mountain Roots Food Project

Mountain Roots Food Project cultivates a resilient and equitable food system in Gunnison Valley, Colorado, by enhancing healthy connections between earth, food, and community. We foster knowledge, teach skills, and provide opportunities that ensure access to affordable, nutritious food that is regionally based and sustainably produced.

Chef Ann Foundation

Academic performance increases in children who eat a healthy lunch each day. This effect is 40% greater among children who participate in the free and reduced lunch program.* Our mission is to provide school communities with the tools, training, resources, and programming that enables them to create healthier food and redefine lunchroom environments.

Ceres Community Project

Since 2007, Ceres’ core program, Healing Meals for Healthy Communities, has created and delivered organic, nourishing meals for thousands of people experiencing health crisis. With an overarching commitment to Food as Medicine, Ceres meals help our clients with serious illness by relieving stress, preventing malnutrition, and connecting nourishing food to health. We use all organic foods, mostly fresh and local, to maximize our meals’ healing power and reduce exposure to toxins. Notably, we are the only meal service that puts youth at the heart of this work.
